Self-assessment tax return time is here again. Many of you have already sent your 2015/2016 tax information to us for completion. If you have not already done so, and you want us to complete your return, please fill in the form below and submit it to us.

If you have already sent your information to us we need to make sure that we have all the details about your income and expenses, you may still need to complete some of the information requested in the form.

The final due date for 2015/2016 tax return submission is 31st January 2017. Failure to submit the return by that date may result in automatic penalties. Interest will be charged on any tax due and not paid by that date.

Please submit the form to us no later than 31st October 2016, this will give us time to check the information and request any missing details. If you want us to take care of your partner's tax return please submit a separate form with their details.

* For those who would like their tax to be collected through their tax code, please note the deadline for submission is 30/11/2016 (Internet submission).

We will be filing all personal tax returns using the internet this year. When your return has been completed we will email it to you for checking. Once you have confirmed that the return is correct we will then file the return on your behalf.

What information do you need to provide?
Employment Income You only need to enter Employment Income if income received from any employment for which we did not process the payroll and Employer's end of year returns for the 2015/2016 tax year.
You will need your P60 / P45 or last payslip for 2015/2016 for each relevant employment. If expenses were reimbursed or you received any benefits from a relevant employment you will also need your P11D.
Self Employment Income This section needs to be completed if you carried out a business as an individual rather than working through a limited company.
You will need details of your income and expenses for the accounting year that ended in 2015/2016. If you need help preparing accounts for any self-employment please contact us.
Bank or Building Society Interest This section needs to be completed if any interest was received during 2015/2016 from banks or building societies. You do not need to include interest on ISA accounts.
You will need your bank / building society interest certificates or statements for the tax year.
UK Dividends This section needs to be completed if dividends were received from UK companies other than your own limited company. If you have recently moved to CDJ Online from another Accountant we may not have received the dividend information for 2015/2016 from your previous accountant. Please either contact us to see if we have the information or include the dividend information.
You will need your dividend vouchers for the tax year.
Social Security Income This section needs to be completed if any social security income was received during 2015/2016. Please include all social security income,including Child Benefits, we will identify whether or not the income received is taxable.
Rental Income This section needs to be completed if any income was received from property during 2015/2016.
You will need details of the property income and any related expenditure. If you need help preparing income and expenditure property accounts please contact us.
Personal Pension Contributions This section needs to be completed if you personally made contributions into an approved pensions scheme. Do NOT include pension contributions made by your limited company.
If you are not sure what your total net contributions were in 2015/2016 please contact your financial advisor.
Charitable Donations Payments under Gift Aid for higher rate tax payers will result in some tax relief. From 6 April 2003 you can make gifts of any amount under Gift Aid so long as you have given an appropriate declaration to the charity. Payments made under Deeds of Covenants made before 6 April 2006, are automatically treated as if they fall within Gift Aid. Deeds of Covenant set up after 6th April 2003 will only be tax effective if an appropriate Gift Aid declaration has been given to the charity.
Do NOT include amounts donated through an Employer's Payroll Giving Scheme
You will need details of any charitable donations made during the tax year.
Capital Gains This section needs to be completed if you have disposed of any assets in the tax year.
You do not need to complete this section if the asset disposed of was your own home (main place of residence).
However, if you made a loss on the disposal of assets, please include the details.
You will need details of the sale price / purchase price and any related disposal and acquisition costs. Please contact us if you need help with this section.
Names/Dates of Birth of Children Please include the names and dates of birth of your children. We will check to see if you are entitled to Children's Tax Credit. Please advise us of any child benefits received by you or your spouse during the 2015/2016 tax year.
If any of the children are not living with you please say so.
Other Information This Tax Return data collection questionnaire is designed to collect all the information that we know is relevant to our clients. However if your circumstances have changed or you are a new client there may be other information that you think is relevant.

  EMPLOYMENT INCOME

If you have more than three additional Employments to report please use the Other Information section.
Enter Gross Income (before deductions) and Tax deducted from your P60 or P45. The Employer's PAYE Reference can be found on the P60 or P45. If you can't find the P60 or P45 you should find the info on the last payslip for the employment in 2015/2016.

Benefits and Expenses from Employment
You will find the information about expenses and benefits received from employment on the form P11D provided by your employer. Enter the PAYE Reference of the Employer in the top row and the benefits / expenses relating to the employment in the same column
Don't forget if the expenses reimbursed were necessary expenses incurred solely for carrying out the employment you can claim, tax relief. Make sure you fill in the Expense Relief section below.

  SELF-EMPLOYMENT

You need to complete this section if, at any time during the 2015/2016 tax year, you worked as a Self-Employed person. If your annual turnover from self-employment was more than £81,000 you will need business accounts prepared. Please contact us if you need help with this.
For each self-employment enter the income, expenses and capital allowances for the accounting period ending in the current tax year. If you are not sure what the correct accounting period is, please contact us.
